This is the prescribed application form under Article 4.2 of the PSPA Constitution. It goes to the Chairman and Vice Chairman through the Secretariat. Have your Bank of Uganda licence, an application letter on company letterhead signed by your CEO or managing director, and the signed agreement template ready to upload.
